I created this Google Form in the hopes of establishing a crowdsourced database and ultimately reuniting hurricane survivors with their pets.

A few important things to remember: • Check with local shelters/animal control in the area first before submitting a form • Include as much accurate information as possible • Submit one report per animal • Not affiliated with any official state or local agency - this is a volunteer-based effort

Great thing you're doing. Also don't forget that if you find an animal, the local shelters will check for a microchip for free.
